It seems to happen a lot when I click on an LED and then press enter. I think I've seen it even when clicking an LED and then clicking OK though. I definitely always hear the click sound, and I think I've even seen the LED turn red, so I don't think this is a case of just accidentally missing the button, or accidentally dragging the mouse off while still pressed.
